Jack in the box

I was reading a book on persuasion (nothing to do with origami) and the author was an ex-magician who wrote about how to get people's attention. He mentioned that he can do a simple origami jack in the box using a dollar bill.

Does anyone know which design this is?

I guess it must be an adaptation of max Hulme's model, which is originally made from a 2 x 1 rectangle. Diagrams at some old BOS and FOCA magazines. If you want details, check http://www.origamidatabase.com/ and type jack in the box as the model name.
