It's a Treemaker'd base. ReferenceFinder is probably your best bet.

I can't/ don't know how to use it. What are the numbers I enter? :?

Use a ruler (or better yet, using your computer) to measure the coordinates of important looking points, where the side of the square is 1. If on the crease pattern, the point you want is 1.3 inches from the left, and the square is 5 inches, then the x-coordinate is 1.3 / 5=.26. Type that into ReferenceFinder, and do the same for the y-coordinate.
