Designing a Model

I am looking for someone to design a model for me. I will pay money for it of course, and all I would require is a crease pattern, or if you are feeling generous, a diagram. It looks to me that it would be simple to design, it's just that I don't have the experience or skill to design it yet ... and it could take a long long time to acquire those skills.

My friend is very sick and I want to make a model for him, so any help would be appreciated. Like I said, I am willing to pay money, so that's not a problem for me.
