Derek McGann has one of the few that are out there. The model is a little past halfway down the page.

There's another leaf insect in the book "Origami Insects I" by Fumiaki Kawahata and Seiji Nishikawa. You can see an image of the folded model here. Again, it's slightly past halfway down the page. Or you can use Ctrl+F to find "leaf insect".

I'm not sure if this classifies as a walking leaf, but it is a similar subject. There exist diagrams to this model in addition to the crease pattern at the bottom of the page, but they are quite hard to find. Besides, this is an easy crease pattern. (Basic frog base with much thinning)
