I don't have any info on the model, but it is stated in the Flickr guidelines that, when posting photos from Flickr, you should link back to the photo page.
Flickr no longer displays a direct link, so it's actually harder to NOT link back.

Maybe you saw the photo on someone else's site, and you just copied the link, so I'm not aiming this directly at you, but that is part of the why linking back is always the best idea - this is the photo page, and the "someone" who folded it was Richard Galindo.
He mentions having recieved poorly drawn diagrams from a friend, so maybe you could send him a Flickr mail and ask politely for them...

I don't know for sure, but I doubt there is a crease pattern.
Satoshi Kamiya has lots of CPs on his site for various models, so if there was one, I think it would be shown. Also, the model is not listed in the normal "Gallery" section of the website, but is listed under "Others".

However, there is another section called Crease Patterns. They are not labeled, though, so it may be difficult to find the right one...
I think you can safely ignore anything with grafts for toes/fingers, and there are a couple which are obviously not it but, even then, there's no guarantee that the CP for the Kabutoceratops is there.
