does anyone know which book this diagram is in? Is it from 1 copy of paper? thanks

That's Fernando Gilgado's Dragon-Heart. Diagrams are for example in Tanteidan Convention Book 13 or OUSA Convention Book 2007.

This very nice model is indeed from one square sheet of paper.
